As Blitz asked for where the missing fail would be: Check the map (it's one of the buttons in every fail screen) to find the missing fail. There you probably need to wait out a timed event. Also, read the portal gun fail text completely. In the sentence "if you want an achievement, click here", click the word "here". Also, there are multiple fail screens where clicking the words does something. In one case it's even clicking single letters to spell out the reference in the fail scene. Also, everything that might be special to someone is worth to click on. Things like donuts, pictures, suns, moons, bagels, ... and people, but people need to be right-clicked.

Hey Blitz, I found out where the last fail is on Henry Stickmin escaping the prison. It's in the chase where you just escape the bathroom with crowbar. When you get to the wall, don't do anything and Henry will run into the wall.
