Personally I'm not too sure exactly where it's all distributed, so I'm not the best person to answer the question. But one thing I've always been very appreciative of is the NHS, where we get free health care. I was watching an interview with Boosie last night and he stated that he had to pay $80'000+ for his cancer surgery and I started to think how crazy the concept of paying for health care is.
