I don't know what you were trying to do with the gun against the tyrant of instinct, but I have 3 infernal wishes right now in my vault that aren't proccing the two shots, I had to find one that would... I think some legendaries are literally bugged where some aren't

Infernal wish doesn't proc until you either go down or you have action skill start anoint
