Debrief with Dr. Choi-Kain (expert clinician/researcher) after her first meeting with Jake -- diagnosed with BPD (Borderline Personality Disorder) and likely co-occurring PTSD (Post-Traumatic Stress Disorder). The main topic is the differences and similarities between BPD and PTSD, and which treatment options may be appropriate at what time -- with a backdrop of Dr. Choi-Kain's first meeting with Jake.

Dr. Choi-Kain's short bio (lifted from the McLean Hospital site):
"Lois W. Choi-Kain, MEd, MD, is the director of the Gunderson Personality Disorders Institute. The institute provides training and supervision for numerous proven treatments, including mentalization-based treatment (MBT), dialectical behavioral therapy for post-traumatic stress disorder (DBT-PTSD), transference focused psychotherapy (TFP), and general psychiatric management (GPM). She works nationally and internationally to expand teaching efforts on borderline personality disorder and its evidence-based treatments and engages in research to study resources for training clinicians who need direction and patients who need access to informed care. With her mentor, John Gunderson, Dr. Choi-Kain developed a training program for GPM and has been expanding its applications.
In 2009, Dr. Choi-Kain developed the Gunderson Residence, a specialized residential program for adult women with severe personality disorders. In 2013, she founded the BPD Training Institute, a major center for proliferating awareness of and evidence-based care for severe personality disorders. Dr. Choi-Kain has also developed training clinics in McLean’s Adult Outpatient Services for treatment approaches such as MBT and DBT-PTSD."

“People aren’t harmful, they’re inherently disappointing “ I agree with this although my experience has been that many people have the capacity to be willfully harmful given the right circumstances

I have found that giving up alcohol (I'm an alcoholic) , getting plenty of exercise, eating a well balanced diet, have helped me get back control of my life. I'm able to regulate my emotions and make sense of the world a little bit more. By developing good habits, leads to perceptual changes, becoming a happier more rounded individual who is more approachable these days. Also never underestimate the benefits of therapy , it helps to develop growth, understanding and gives you tools and coping strategies. Hope this helps someone, stay safe people❤

You sounds rich enough to have the luxury of doing all these good things for you. Good for you. I don't. Also I dont underestimate the benefits of therapy, it is the price of therapy that is insanely overestimated. Therapists needs us to pay their pools, or in the case of this one: her hairdresser

I'm looking so forward to this upload. Thank you. I'm suffering so badly, waiting for help. Dying on every hill. As the waiting list for clinical help... Therapists... is long and our healthcare system here so fractured. I've no choice but to self soothe with self help in the meantime. Gratitude 🙏 Ontario, Canada 🇨🇦✌️

So helpful. From her describing Jakes present mindset and communication patterns, you get a birds-eye view of the maze that people find themselves in, feeling distracted and situationally trying to placate others to fend off harm. Humane discussions. Thank you.

I think validation that they discuss is addressing a lot of the gaslighting that came with abuse. Being told you aren't allowed to be angry/upset/sad by abusive parents and that your feelings don't matter and then the sort of break that is at the center of BPD, which is the understanding that as a child deep down on some level you know that what is being said/done is wrong yet you inevitably end up internalizing these messages. The most important thing is clearly validating that those early experiences were f'd up and that internalization is the what needs to be constantly addressed and re-framed combined with having new experiences that prove otherwise. Although for Jacob and what I think the psychiatrist might not quite understand is the extent of unhealthy relationships within the gay community which result from a lack of practice relationships and societal acceptance. As in, it's mostly probably going to be short-term relationships that just don't work and thats regardless of BPD... it's due to the homophobia and growing up gay. I'd say most therapists don't touch this bit with a ten foot pole because it just further muddies the water and makes treatment feel more complicated, and yet is what will happen in reality.

I felt quite a connection to Jake when I watched the session with him. It's nice to hear Dr. Choi- Kain say she thinks he can have a better life with the work he's doing with his therapy. I have some similarities with Jake from listening to the session and now with this debrief. I'd really like to find out what my one to one therapist had to say about me when I did DBT with the team I saw. I'm from the UK and had my treatment via the NHS. I wonder if knowing how she spoke of me to others, if that would help me with the difficulties I'm having now. I'm sure she liked me and cared about. Which is very scary to think and believe as it doesn't fit with how I see myself. I'm going to watch this video again as I think there's things Dr. Choi-Kain said that I may be able to use myself. As always thanks for the upload

Have been so looking forward to this conversation. Compassionate validation is the main tool I've turned to in my recovery. It kind of instantly gives me the benefit of a larger perspective, because my default instinct was I deserved the way I felt and the things that happened to me. Looking back on it, I did my best to teach my ex husband how to care for me, if he had done so, I wondr where we'd be now. As it was, giving compassionate validation, he'd made clear that was the LAST thing I'd ever get. He wanted me to stop talking to him about it, he wanted me to let him logic or dismiss my concerns away. Yikes! fNow I've learnt how to provide it for myself and there's no stopping me now. :). It was interesting to hear takes on what can happen in a person who expereienced severe trauma from a young age. My experience has been once you start figuring out the main issues, you see tendencies all over the place inside. A year ago I would have said BPD was single handedly the biggest and most difficult thing I was dealing with, and that once I got it under control I'd be happy as a lark. But, the trauma that led to BPD also led to some ASPD traits that flared up out of the blue, and let me see how I have APD traits near the core. All this I never expected, the same as I never expected to find ASD. Now that I am managing BPD and ASD, I'm learning all kinds of things I never was able to wrap my head around before. Not just emotional stuff, but complex geometry, etc.

I’d really love to hear more about the situations where it’s unsafe to enter therapy because of the possibility that the therapeutic relationship actually causes harm. Never heard this mentioned before. What can you do if you’re in that situation but wanna heal. I was in a re-traumatising therapeutic relationship and I feel so angry at the therapist for how that all played out. Any advice for seeking therapy? Or insights? Wouldn’t almost all BPD have this problem? Why some more than others?
