Why you didn’t use has been in the following question “ How could the accident have been avoided” Which one is correct has been avoided or has been avoided? And why?

It's a good question! After a modal verb, we don't change the verb that follows it. The verb that follows the modal should be in the infinitive without 'to'. Have a look at these examples: The accident has been avoided. The accident could have been avoided. How could the accident have been avoided? We hope that helps answer your question!
