Brady Universal Valve Lockout

the procedures for ball valve and butterfly valve are ok, but the one shown for the gate valve (e.i., the way the cable was run through the hand wheel spokes) leaves the valve operable in the open direction up to and maybe more than 180-deg - not a good isolation lockout.. lockout integrity dictates that the valve hand wheel should not be operable in the unsafe direction by more than 10-deg of rotation.
