"Kernohan notch phenomenon is an imaging finding resulting from extensive midline shift due to mass effect, resulting in the indentation in the contralateral cerebral crus by the tentorium cerebelli. This has also been referred to as Kernohan-Woltman notch phenomenon and false localizing sign. The integrity of the crus cerebri and its descending corticospinal tracts is disturbed, and a contralateral (to crus cerebri) motor deficit is produced with a deteriorating level of consciousness. A hemiparesis ipsilateral to the expanding mass is known as Kernohan phenomenon, which is a false localizing sign." This is taken from radiopedia. So, hemiparesis or CN 3rd palsy?. it's both or one of them. ""Uncal transtentorial herniation Early herniation ---> ipsilateral blown pupil ( unilateral CN III compression), controlateral hemiparesis. Late herniation ----> coma, Kernohan phenomenon (Misleading controlateral blown pupil and ipsilateral hemiparesis due to controlateral compression against Kernohan notch)"" Step 1 First Aid.

and of course that uncal herniation can lead to PCA compression and subsequent occipital lobe infarction.

3rd Cranial Nerve palsy in uncal herniation is very rarely false localizing..
