My mom diagnosis with BC.. I asked her oncologist about her stage she said it seems stage 2 and accurate stage will be known after surgery..oncologist said her tumor is flatted ( something like that she explained) so she need chemo before surgery so that surgeon can remove it easily...Now my confusion is If chemo is given before surgery will not chemo reduce the tumor size & kill the cancer then how stage will be accurate??( We already given her 3 chemo & we feel her tumor size is getting smaller)

Thanks for writing, and your mother is fortunate to have you looking out for her. You are correct that it will not be possible to assign a pathologic stage when chemotherapy is given first. The stage following chemotherapy is even more important. The way stage is designated on a pathology report is by putting "yp" before the stage. "Y" refers to "neoadjuvant" (or "primary") breast cancer. The "p" refers to pathologic stage. The amount of disease left after chemotherapy helps determine if additional treatment will be useful.

I would argue that waiting to find out what your biopsy and MRI have revealed (over a Labor Day 'weekend'), is its own form of torture.

My wife has a 2.7cm tumor, (estrogen positive) and 1 node positive (they checked 3)...would this be a grade 2? We meet with Dr in 5 days to discuss further. We are just a bit anxious....great video by the way

Great question. Given what we know, this is a Stage II (2) cancer. Stage tells us where the cancer is in the body and how much there is. Grade actually refers to something else--the appearance of the tumor under the microscope. You will most likely be told the grade, ER, PR, and HER2 when you see the doctor. Keep coming back. Ma'am my mom did her surgery...the report came breast cancer with axillary clearence left- infiltrating ductal carcinoma,NOB, grade 2, stage - pT1N0Mx ,LvI, PNI- not identified.... Now what does it mean??? (ER & PR- positive, fish test her2- neg)

It sounds as if there is no LVI (lymphovascular invasion) or PNI (perineural invasion). This means that the cancer cells have not invaded into the blood vessels or nerves of the normal breast tissue that surrounds the tumor.

Hi there! Forgive me if I’ve asked this already or if you’ve already done a video on it- I am fresh from a mastectomy (still with my drains and everything) and I’m wondering if you could talk about microinvasive breast cancer. The reason I think I may have already asked you is that this was suspected when I had the biopsy done. Today, the path report from my mastectomy came back and they found 8 foci of microinvasion. Because I have HER2+ DCIS, I think the concern is that the microinvasive cells may behave more aggressively, if I’m understanding it right, so the surgeon wants for me to speak to a medical oncologist. I’ve since done some reading and it seems as though there is a lot of work being done to determine the implications of various aspects of microinvasive disease (number of foci, receptors, size, etc). I gather this is intermediate in stage between DCIS and Stage 1 -or do I have that right?

You are correct that a great deal of micro invasive disease can "upstage" the cancer from DCIS to stage I, but it depends on the extent of the micro invasive disease. Your team can go over the specifics in your case. Wishing you the best.

Thanks for the detailed video. Can the cancer metastasize to other parts of the body even though no lymph nodes are involved?

While positive lymph nodes are associated with a higher risk of later metastatic spread, tumors with negative nodes can also metastasize. We offer chemotherapy and endocrine therapy as well as targeted therapy based on things such as the tumor size, the grade, ER and PR status, and HER2 status.

In general, the pathologic stage is more helpful. In people who receive chemotherapy before surgery, we use the clinical stage to make decisions about radiation therapy. In other words, if someone had a clinical T3 tumor that responded to chemotherapy and was a pathologic T1 tumor (residual disease after chemotherapy), we use the pre-chemotherapy size and would offer radiation therapy.

I hv one tumor of 4 cm and spread to one node on the same side. Just want to knw what stage it is ?my pet scan didn’t write my stage, they just told me nothing to worry as it didn’t spread to other parts of body

A tumor that is 4 cm is a T2 tumor. With one positive lymph node, the N category is N1. Combined, this is a Stage II (two) B cancer. Stage is not the only thing we use to make treatment decisions. We also look at the estrogen receptor, progesterone receptor, and HER2 status as well as several other things.

Luminal A cancers are positive for the estrogen receptor and negative for HER2. Luminal B cancer are positive for both estrogen receptors and HER2. Luminal A cancers are considered low-risk. Luminal B cancers are more "active," having in general a higher grade.

Thank you for writing. This type of cancer is treatable, and the goal of treatment is cure. While the risk of the cancer recurring is higher than in lower-stage disease, I have had many patients live for many years without recurrence.

In someone who is thought to have Stage 0 breast cancer, also called non-invasive breast cancer or ductal carcinoma in situ (DCIS) based on the biopsy, it is possible that invasive disease will be found when the tumor is removed during surgery. In that case, it would not actually be DCIS. If someone does indeed have only Stage 0 breast cancer (confirmed by surgery), it is exceptionally uncommon for the cancer to recur (come back) in other parts of the body. We're bringing up two separate things in this response to make sure we answer your question. Let us know if you need more clarification.

This is a terrible situation that faces many people. Hospitals will often provide care for people in such a situation. If you live in the United States and have no income and have cancer, Medicaid insurance is a possibility. There are also grants that can help pay for some of the costs of care. As you are deciding where to get your care, ask what options they have for you.

Triple negative breast cancer is generally treated with chemotherapy. Chemotherapy can be given before or after surgery. This must be a difficult time for her and your whole family. We are wishing you the best.

Wow, watching them grow every week until they grew by two cups is definitely satisfying, I went with what I commented about and after the first 20 days or so I went up by one cup! I simply go'ogled the latest by Mika Klopsworth and now they’re fuller and firmer than I expected!
