What timing... I'm a first-year Master of Athletic Training student at Penn State and we literally just had our spine boarding practical last week. I also plan on obtaining my EMT cert next semester. I get to work at a nearby highschool for the semester as part of the program at Texas A&M and we had an incident early in the football season where we had to send one of our players to the ER. Thankfully it was more as a precaution and not because of the severity of his injury, but being thrown right into a case like that really makes you appreciate all the medical staff involved with sports who are well-prepared for these moments. @@RuralHousewife The NFL doesn't need ORs in the stadium like bullfighting. Bullfighting injuries are from being gored or trampled by a bull. Those require immediate surgery. Guys could die from being pierced by a horn, or bleed to death internally by being stomped, so an OR makes sense. With NFL, vital organ damage is rare, and guys aren't getting gored. The most common football injuries are impact injuries (concussions and broken bones), orthopedic injuries (knee, ankle, elbow ligaments), and muscular (groin, pectorals, bruises, sprains). Many of these do not require immediate surgery. The most critical to treat are possible spinal injuries to prevent paralysis, and there are protocols for stabilization until a specialist is able to treat them. All other injuries can wait for MRIs.

The most life threatening injuries that happen in the nfl are brain/spinal injuries. Treating that kind of thing correctly is going to require more imaging and testing than what would be available in an OR.

What's really amazing to me is that while the NFL is the biggest 32 teams, they're at the top of a sport that's played at 20,000 high schools and thousands of colleges, as well as kids starting even younger and other amateur and semi-pro leagues. Those smaller venues all have far, far less capacity to handle injuries like this.

Thank you for a detailed explanation. So few outside of emergency trauma medical training understand what is happening so it helps to have someone calmly explain. Sometimes knowing more causes us to speculate more about possible injuries, but care for players on the field and enroute has advanced greatly in the last ten years.

Hi Dr Surttere i really love you explaining what we are seeing. I a a retired paramedic. I can tell you that cutting off pads is kind like a power trip but we still need access to the chest ( Remenber Hamlin from the bills ). Now, when I went to school, we did not take the helmets off. We carried a Philip's screwdriver in our madbox to take off a face mask. But we did not take off the helmet that was left in place and they were mobilized with the helmet, but I can guarantee this. That was the report of helmet hitting and then bouncing off the ground in my books automatically bought a spine more than Haller. No matter what the patient said.

Main thing they stressed in the EMT portion of the Fire Academy is that in trauma, once you take C-spine. You LIVE THERE! That is how your home, your office, your bedroom. That person is Joe your best friend, your damn sibling. You do NOT release c-spine until they get a collar and secured on a board. That persons neck is your HOME!

When I was a young medic I went to a ladies house on a call and she was laying on the ground. So I walked up and took C spine as she was laying there. She was talking but not moving. I found out while talking to her that she wasn’t moving because she was already a quadriplegic. I was so unsure what to do in this situation so I kept C spine anyway. My crew laughed at me later that I refused to let c spine go until we got to the hospital and the doctor told me I could stop 😂 but yea c spine is very important.

@@mystimallowYes and no. The hit into the defender can’t be stopped. BUT on natural grass there is quite a bit more cushion for the follow-up force that could change the outcome of this. Natural grass and drier are FAR softer than turf on concrete. So yes, all concussions and concussion possible situations can be potentially improved with real grass

It’s the way the game has changed. Back in the day QBs wouldn’t put their receivers in that situation. Because of the rule changes nobody fears going over the middle so QBs don’t fear throwing it there.

@@goroadachi9489 What exactly is the lie? They’ve changed the rules and receivers don’t fear going over the middle like they used to. It gets talked about all the time. If receivers aren’t fearful of going over the middle, QBs aren’t going to be fearful of throwing it there. I never said that Tua tried to throw him into a bad spot. But QBs, in general, are less concerned about throwing over the middle because defenders can’t lay out receivers the way they used to be able to. Back in the day that throw would’ve been low and away from the defender, not leading the receiver into the defender, but the protection of receivers from the rules has made ball placement less of a concern than it used to be. Obviously they’re not trying to throw their receivers into vulnerable spots but they’re less focused on protecting them than giving them a chance to catch the ball. 20 years ago that ball never would’ve been thrown in that spot.

If they had to access the neck and to do it completely, the helmet had to come off and the most appropriate thing to do with the pads is to cut them away so that you can maintain a neutral spine. It's more difficult to get that neutral spine if the helmet is removed and the pads stay on. So it may just be that. Another reason could be that they need to ensure a neutral spine and the pads make that very difficult when rolling someone on their side to assess the back. The monitor can be placed in other locations to get your initial trace. I had to do this most often while in the car with patients who were being extricated and complaining on non-traumatic chest pain. Ideally though it should be place on the core to decrease interference while going down the road. The IV can be obtained anywhere on the limbs and with orders on the neck in some places. So this wouldn't necessarily be a reason to remove the pads or jersey.

In high school I made a text book tackle. Hit the running back square with my shoulder and head in front of chest. The hit in that area is just as bad as head to head contact. Side impacts can be worse. That said I finished practice unknowingly. What tipped off everyone something was wrong is that I was looking for car keys. I didn't even have a DL. I was 16 at the time. Don't remember anything about practice or after. I just remember waking up the next morning in the hospital. Went to see a specialist, and he advise to not play football anymore. Amnesia is bad and you try hard to remember everything and you can't. Only way I know, is what everyone told me.

I think it's standard practice for the Paramedics to connect a patient to a monitor for watching vital signs during transport. As a respiratory therapist who works at a trauma center all EMS patients come in on a monitor. Part of what we do when the patient comes in is switch from EMS monitoring to the monitors in the ER. I'm sorry this player got hurt but he seems to be getting the best treatment and care possible.

Excellent explanation. Interested in your thoughts about an issue in Cleveland last weekend (I think it was in the 2nd quarter). A player was hit and seemed to have at least left arm extensor posturing. Staff came on to evaluate. The decision was made to let him get up and walk off the field, even though he was still unable to walk on his own. If you get a chance, can you review that sequence and tell us what you think? I was encouraged when the video showed that the hit didn't directly compress the spinal column downward...that is where you tend to get permanent spinal cord injuries. The hit was to the right side, which caused a whiplash effect. I'm sure he's concussed, and likely has some spinal cord bruising but, fingers crossed, he probably avoided the "tragedy" someone here mentioned.

Thank you for this really informative breakdown on such a scary-looking situation. In a situation like this, when they’re afraid to disrupt the head and neck, how do they remove the helmet? I understand how the facemask can be taken off, but there aren’t visible screws for the helmet itself.

This is why it’s impossible to play defense. Try to go low to avoid head and neck but receiver goes low too.

Having never played football myself, I am genuinely curious abouty something. How do you remove the helmet from a player on whom you are trying to stabilize their neck?

Athletic Trainer here, when stabilizing the spine you would have another person assist from the front in stabilizing the cervical spine. Since the facemask has been removed, the helmet can then be tilted off of their head and removed.

It is a bit weird with all the safety talk, that the NFL does not stress proper falling techniques and positioning of the head and neck during hits. I grew up playing football and they stress proper tackling form, but I never had a coach cover proper form for receiving hits or falling. I learned that from wrestling, judo and tumbling. This hit was different as he wasn't looking and wasn't able to set himself before getting hit. However overall, week in and week out if you want to see a highlight reel of how not to fall or position yourself to minimize the chances of a head injury the NFL is the place to look.
