What I do not understand is why are doctors not being made responsible for medication errors just as nurses?

As much as nurse made an error with medication but Doctors it their responsibility to check patients allergies before prescribing medication because it always written on patients prescription chart.

It is important to avoid interruptions, and also to know the patient allergies... After giving medication a nurse should monitor the patient for possible reactions.

When at work one needs to focus on the duties assigned, paying attention on the basic fundamentals. Identification, double checking and avoid interruption. Eye opening, and a refreshing reminder of basic fundamentals of the profession.

The font/print size on IV medication labels that nurses give is extremely small. This has huge potential to cause medication errors. Why is nobody fixing this problem?? It's unbelievable and continues year after year to be a MAJOR problem.

Better to be safe than sorry.. the lesson is that you can never be to carefull when it comes to medication and your patient. Better to control medication with RN or doctor.

A Nurse didn't follow five rights for administration of medication ,which is very important ,she just introduced herself ,but no Identification of the patient, Could happen maybe she gave the right medication but on the wrong patient,also checking with a second person is very important because some Dr's hand writing are not clear

This comes to show that even in the midst of just a slight distraction.mistakes can be fatal.she did not do the proper checking and she was interrupted in the process and also did not check for reaction there on after.

Doctor refused to co_check with the nurse. It lead to the wrong giving of medication.

This nurse didn’t follow any protocol If she gave that penicillin content antibiotic slowly she could have caught the reaction She didn't follow any procedure with protocols What makes us think ,what Dr has written is right,?

Wrong medication has been given .nurses don't greet the patient she interrupted the other nurse .allergies has not been checked and patient was allergic to penicillin.

Nurses were so unprofessional, interrupting the one giving medication without considering the patient's presence.nurse dd not check allergies and didn't insist that Dr confirms medication with her.after giving medication she didn't check for reaction.
