This is a mu opioid receptor partial agonist most commonly prescribed as Suboxone which is the combination of buprenorphine and naloxone. Addiction treatment is still filled with stigma as a physician who treats a lot of substance use disorder, I want to bring more attention to this life saving treatment.

But overall this medication saves a lot of lives, and helps people get out of a drug addict life style. Even though you're still technically on an opiate and dependent on it, at least your not scaping change up for drugs, or having to call some sketchy drug dealer. It takes the immense financial stress, and guilt away from the equation. Its much better than methadone because methadone makes you feel like your going to get your drugs every day, it keeps you in that mentality.

So a Psychiatrist in sneakers convinced you. It would be much simpler if you recommended Kratom. It’s way better and when I come and see you and you go on your laptop to see what prescriptions I had filled Kratom will not be on there. Great right. Just remember when you put someone on Suboxone every Dr/ NP/ PA-C can see it . Then there is a bias. Plus the Pt keeps their teeth. So many lawsuits against Suboxone.

Since then it’s been better and it’s amazing not having to take something daily anymore. The hardest part of addiction was constantly thinking of when I could get my next fix, and worrying about having enough supply on trips/overnight stays at friends places. No longer is that a worry of mine anymore. And no more ups and downs with the strips! Just feeling normal and well 24/7. I’m excited to get my next two, and also last two shots, to see how my experience is with getting off of bup. Goes for me. Lots of people have had extremely great success with the 300/300/100 and then nothing. No withdrawals reported

They need to do something about snorting the medication. No matter if you have strips or pills, you can snort them, and the bioavailability of the naloxone is so much lower than the buprenorphine, the medication ends up hitting a lot harder. The bioavailability of buprenorphine nasally through the mucus membrane its closer to 50%, vs sublingual witch is around 20%. This means people can abuse the medication and get high. I don't know how you could make it so it cant absorb through the mucus membrane in the nose, and still dissolve sublingually. Because sublingually its still using the mucus membranes in your mouth. People do this in jail. They dissolve suboxone strips in water and snort them, my friend told me about it in detail. The abuse of this isn't really being talked about much, but its happening a lot. I personally love this medication and it saved my life. I just think they should do something about the sublingual pills, and strips not being fully abuse proof.

But I messed up bc I stopped taking my suboxone to do this one drug and fell off for 3 weeks.... but now I started my suboxone again bc I felt like I was losing control of my life again. In order to start suboxone we usually prefer the patient be in mild to moderate withdrawal prior to starting. We would want to have a COWs score of 11 or 12 before doing the induction. I'm speaking about this in terms of addiction treatment I'm not as familiar with pain management as it's not my speciality. The withdrawal symptoms should subside in 30 minutes or so after starting the subxone but some additional adjustments to the dose may be required over the first day. It's not the greatest medication for the treatment of chronic pain. In this case it really depends on the details of your friends case and why this medication was chosen by their doctor. Neuropathic pain does not respond all that well to opioids and the mechanism is thought to involve an inflammatory process which these medications do not target. I would talk to the doctor and explain what's going on

@@ShrinksInSneakers my theory is that there must be something about thebaine derived drugs (OxyCodone, Oxymorphone and Buprenorphine) that create a unique feeling that other opioids don’t. Methadone was this awful heavy feeling. OxyCodone, my drug of choice, gave me energy and then around 80mg or so I would get that typical nod. But the euphoria and energy was unlike any other opioid esp the way it improved my mood. When I went on Buprenorphine I had just tapered off methadone and was opioid naive as my goal was to try abstinence. I couldn’t do it and when I tried Buprenorphine it literally felt like OxyCodone used to for me. Except the euphoria wore off much faster. By the time I was at 24mg (a few months in) there was no more noticeable high. The improved mood and the craving control is still effective ten years in on 14mg. Methadone was like taking an opioid and getting only the negative side effects. I also experience no issues w Testosterone or constipation at my dose. Something about the thebaine alkaloid though. It produces much more stimulation than typical opioids. It’s also where the antagonists are derived from. So many heroin users don’t like Buprenorphine. The more tools the better.

Great information! Question though, I never get straight answers for dosing my medication. I take sublingual tablets, do I press my tongue down on the tablet once I set it under my tongue? I’ve heard some people say to clamp your tongue down on it, others say to avoid it with your tongue. Then I’ve even read just to place medication under tongue and then just relax your tongue. This is a good question, When you take a sublingual medication its absorbed directly in the blood stream and bypass hepatic metabolism. It should not matter if you press the tongue on the tab. I personally prefer the film strip I think it dissolves better and has better bioavailability but insurance can get in the way. Unfortunately it's not recommended to crush, chew, or swallow the sublingual tablet. See if the film is an option with your doctor and watch for overmedicating as the higher bioavailability may require a dose adjustment when switching to the film.

Yes... I have moderate to severe spinal stenosis. The shots only work for a couple weeks to a month. 4-6 mg's of suboxone has done wonders for me. And it does not get you high. It keeps me in pain free mode for the most part. Unless I ride my bike a little too hard. But it works very well. I found out because I started to use fentanyl for pain since it was readily available. I quickly realized how dangerous this new powder fentanyl is. So I got suboxone to get off fentanyl, and what do you know it fixed my pain and my drug habit. And back pain is no joke. Just remember, you don't need the high dose when used for pain. I find 6mg's being the most needed. And I take it 3 times a day for pain. Or if I do 4 mg's I take it morning and evening. I just add a lunch when I do 6.

I injected fentanyl and heroin daily for 20 years, and I can get well off 4 to 8 MG. Suboxone doesn’t get you high, or make you feel anything at all, besides withdrawal relief. Because of the ceiling effect. One time I took 60 subs at one time, they were subutex so I shot them up. and it felt the same as taking one. If you wanna get clean, take subs once or twice a day, and relax. Get out of the pill seeking lifestyle. Don’t hang out with people who use. I keep hearing people say that you have to go to the clinic daily to get methadone, but not for suboxone. Here in Canada they are treated the same way. You have to have clean urine samples for a month to recieve a take home dose of both suboxone or methadone. I have all my take home methadone doses and only have to go to clinic once per week. You gain one take home for every month of clean samples provided for both substances. Only differnce it the naloxone in the sub, that blocks your receptors.

I feel like I still got just as high as if I hadn’t been taking Suboxone maybe a little less high which very possibly could just be in my head because the medics tell you it’s supposed to prevent you from getting High so your expecting it to deter your high to some degree. When I weened down my suboxone dose I went from 16mg everyday and within just 2 weeks I got myself down to 2mg a day then shortly after 1mg daily. At that point I stopped taking the medication all together and went into withdrawl very quickly after which I figured might last a day or 2 after day 5 my symptoms have only got worse. So I got back on suboxone and had to go back to using 16mg a day to relive my symptoms do you have any tips to either prevent any withdrawl once I get back to down to 1mg or if I cannot and have to fo through some withdrawl no matter once when I stop, how many days total do people generally withdrawl through the during the suboxone detox? The symptoms are very intense and uncomfortable. It’s not the same symptoms as fast acting opioids withdrawls. When I was detox suboxone I had severe stomach pains ,constapation, stuffy nose. Serve muscle pain and weakness a lot in the knees severe anxiety restlessness twitching shaking toss and turning restless moving of my legs hot flashes lack of motivation tired wanting to sleep no appetite whatsoever couldn’t even bring myself to take a bite of anything. Ir sucks… I’m sick of having to rely on this medicine everyday. I hate waking up in withdrawl and having to take a tablet right when I wake up and I don’t want to have to hide it from everyone or hoping for workers don’t find out or my boss finding out anymore i want to finally close this awful expiernece of opioid addiction and recovery being sober from opioids after being an addict is one of the best feelings! To kick to Suboxone and officially be free from all symptoms and all medications and be officially free from the control and grip the opioids have on you. Also, the suboxone shot is way more common now and it has helped many get off the subs. I know someone who changed from Long term methadone to subs then the shot and now she’s off it all. The shot has been working wonders by what I hear. But anyway in jail they divert the subs all the time and they love it so it definitely gets them high.

If suboxone works well for pain management why aren’t people being prescribed it for acute conditions? Are they giving suboxone post surgery? In the ER for broken bones?
@ShrinksInSneakers
@ShrinksInSneakers Жыл бұрын
It's more in chronic pain with comorbid opioid use disorder but honestly I'm not a big fan for pain. I would say opioids are best reserved for short term acute pain as you stated for surgery or acute injuries. Even in those cases if you can avoid using opioids even better.

In some cases people feel that buprenorphine improves their mood. In some cases people are looking to just avoid withdrawal and buprenorphine is available. Either way we need to be mindful of the potential for abuse, but it's certainly not an issue for everyone. A few points refuted by a drug user: Injecting it does not in fact cause withdrawal to be precipitated, no feelings of the nalexone whatsoever, this is I’m sure very harmful for the veins though. And strong enough heroin and fentanyl can easily ‘blow through’ the suboxone, getting you high regardless of the bupe already in your system.

What about the chronic pain that the Suboxone doesn’t take care of your OK with your patience suffering whatever happened to first do no harm main goal in your life is to make sure there’s no addiction would you rather have your patient addicted to opioids and able to live a normal life then be in chronic pain and barely able to get out of bed pick a or B I know I’m not saying pick what’s best for you because we know what’s best for you not your patient

Even with a slow taper? How long were you in subs or still on? I been on them for a week now. But I want to not be on it long term I’m just trying to get rid of my methadone withdrawals I been on 60mg and 1mg xan for 14 years. This last week has been the worst withdrawals I have ever had in my life. I always hear the sub withdrawals horror stories tho. But I see many people going to low .5mg tapers and finally get off. I guess it all depends on the person?

Addiction treatment has not been the best over the years. we all need to do better to reduce the stigma and offer these treatments as we would any other medical treatment. In the U.S. the X waiver which was required to prescribe suboxone in the past has been eliminated hopefully opening the door for increased access to treatment. Appreciate the comment

I just started suboxone yesterday because my doctor wants me off the pain medication. I’ve been on it for a LONG TIME now… I had an accident in 2002 that flipped my world upside down. I don’t plan on taking suboxone a long time. My plan is to take it for a few weeks…If I can take it long enough to prevent withdrawals, yet not long enough to be dependent on the suboxone at the same time! I’m taking quarters of my 8mg pill… I dont want to take it for a long period of time. I just want to take it long enough to get me off opiates. I’ve seen people take it for years, but I don’t think this should be a long term medication. I can take 2mg and it makes me SO SLEEPY!! BUT I feel great! It’s helping with the pain… I do think it’d be better for short term recovery though. If I don’t take it long, then I won’t get physically dependent on that too!! Has anyone else done this? I’m curious about this method. I’m confident that it will work… I just have to take as little as possible to get me through this process. Different people need different techniques, and I applaud everyone who’s getting off prescribed drugs and illicit drugs. Everyone’s different and that’s why I don’t like it when people judge others. I don’t expect people to do it the same way as me. IF I’m successful though, it may be a breakthrough experiment! I just wish I didn’t get so sleepy from it! 😂

I understand the harm reduction model..but for those of us who weren’t heroin addicts and working and had families depending on them were also affected. The medical crackdown on Oxycodone/OxyContin here in Canada truthfully ruined my life. Taking Oxycodone was a game changer for me. I took it for nearly 8 years responsibly (sure I was physically addicted) but it allowed me to work and had the bonus effect of removing my social anxiety and depression to the point that I had the most successful years of my life (in every aspect) Once my doctor wanted to take me off, I had already committed myself to project that I would’ve never started if I had my previous social anxiety and pain. Dr sent me to a pain management/addiction Dr who convinced me to switch to suboxone..but after few months and wishing to return to Oxy wouldn’t do this (despite other in clinic that she still prescribed Oxy to) I’ve been on Subs for 7years now..and technically I’m still taking an opiate but all ‘beneficial’ effects are blocked..leaving me flat, anxious and depressed. And life’s gone to crap..not to mention being in massive debt from not being able to complete the large project. I feel sorry for my wife and 14yo daughter because I’m no interested in doing anything. I don’t find enjoyment and the debt we’re in due to the fallout is constant stress.sleepless nights. If only I was allowed to stay on Oxus until my large project was completed like I was asking..life would be so different. But what do I now I’m just an Architect…Doctors know better hey? I understand that many were misusing. But truthfully, in my case..If the crackdown never happened, I can honestly say my life and my families would be 1000 times better. And I know there are many who turned to worse things after doctors cut them off

This is far to complex for me to comment on and I’m not your doctor so I’m unsure what the shared decision making process was for your treatment. I have seen studies where suboxone improved depressive symptoms. It’s a partial agonist not an antagonist so all the beneficial aspects on mood for example should still be in play. I hope you find a doctor who is willing to work with you on a better plan that is most satisfying. It really is a complex topic and like I always say everyone needs an individualized plan
