Why sometimes when I pierce with the calf on gun does it not go right through the ear and into the backing at the back?

Ive had the same issue. On the left ear of my patient, the post of the earing poked through the skin but didnt locked with the backing. The stud got stucked with the gjns adaptor. On the right ear of my patient, yes it did lock with the backing but the stud still got stucked with the adaptor of the gun. I had to pull the firing pin then remove the adaptor from the earing to release gun
