@@lukewalker3 for me it was chapter 3, if you haven’t got the achievement you can look at the chapter select and it will show if you are missing any of the collectibles. You can then replay the chapter and find the collectible. Hope this helps!

For when you unlock the bridge on the first chapter, the order of the symbols is behind the mechanism you turn around. You can see the pictures of the sun, birds, mountains, a river and fish swimming. That's the order 😁

Glitched achievement is I'll make a note of that. For collection of the puzzle notes but I have 100% on all chapters 😕 the glitched 1 left now on bards tale is old school bard because chapter 3 random unlocked lol
