Dr. Jorge, in my opinion, Canada offers a province based pathway because they are short of doctors and they want to build their research capability through IMGs. The expectation is that you continue academic career but if you chose to do clinical practice after going through the process and have an independent license with no restrictions, then you should be able to!

As a nephrologist from Brazil, you can apply directly to do a clinical fellowship in General Nephrology (or advanced nephrology fellowship if you are eligible) at Canadian Universities. Once you get successful in this, then you look out for options for a job under the province-based specialist pathway as I have explained in this video. I personally would approach this by applying for fellowships and not focusing on exams at this stage. These are things that are not advertised. I know this by experience & being in the system - hence the reason for sharing the 'IMG Secrets'. Yes, the process is similar for all IMGs with specialist training from Brazil or other low-middle income countries.

Hello Dr.Rajeev, thanks for your informative video. I am a board-certified dermatologist trained and working in Taiwan ( 4 yrs of dermatology residency training + 3 yrs experience as consultant) . I am thinking of pursuing the Practice eligibility route (PER). However, i am confused by the following 2 issues: 1) how strict is the royal college in determining "Time and content in the specialty training must be substantially equivalent to the Royal College specialty training requirements" ? Since its never possible for 2 different countries to have the exactly same training time and requirement, even the jurisdictions in the approved jurisdiction route differs greatly in training requirements. Have you seen any successful experience in be eligible to write the royal college specialty exam theough PER? 2) If I managed to pass the royal college specialty exam, is it possible that I would have very little chance of getting the requested "final 2+ years of practice in a continuous practice location in Canada"? I am worried since you have said in a previous post that there are very limited work positions which are highly competitive in the field of dermatology in Canada. Thanks in advance for your help~

To state in a simple way, the sequence is medical training, then residency and then fellowship! You could do residency in Canada or any outside country. Both will make you eligible for a fellowship. If residency is from Canada or USA, then you will get a full license, if residency is from an outside country then you have to work your way after fellowship through different routes for the full license. Thank you Dr. Munjur. Are you asking would you get Canada PR based on your MRCOG? If so, Canada provides PR based on a point system. So Education wiht a master's degree or PhD equivalent will score high. My suggestion is that to try getting it through Canadian Experience Class and it really makes a positive difference based on what i have seen!

Hello Dr. I am Pathologist in Iran where pathology residency is 4 years long and as i noticed accepted minimum in Canada is 53 months. I am assistant professor with 5 years experience, will my academic position compensate the shortcoming of education schedule to apply for PER ? I mean the 4 or 5 months that my education have been shorter than Canadian pathology residency? Also, the other route that you mentioned in the end am I eligible for that?

In general the training period should be the same. They may run an exception for outstanding candidates. To apply for PER, there is considerable expense. To me this appears to be a tricky situation. If you are ok risking that fees, then you could apply to see if they will accept you into PER. I have the same feeling about SEAP. The Royal College pathways are in general are more favorable towards those IMGs trained form High Income Countries. In your position knowing all i know, i would personally explore the fellowship options in Canada and potentially province based pathway because of a 5 year experience or alternatively the USA

Ophthalmology is a difficult branch for IMGs to get in but i have seen this happen. A great option will be to get in as a Research Fellow/Clinical Fellow and work for a few years and get Canadian experience to get into the system. The other option (not so great in my opinion) is to become Canadian PR and move to Canada and then explore the options

Dr. Pream, I am not surprised. So does this happen to most other IMGs in your situation? Once you are already in Canada it gets extremely difficult (paradoxically). You are on the right track by trying clinical and research fellow options. Explore less-known hospitals or cities! Try to volunteer. Make your application strong when you apply! If your long-term plan is to stay in Canada then start preparing accordingly for that too!

I'm a nephrologist from Taiwan and I've been doing research on the practice eligibility route for months. Could you be so kind as to tell me how could IMGs get their clinical experiences for 2 years in Canada? (This is the issue I couldn't figure out after searching for so long a time.) Would you recommend to get the eligibility for RCPSC's exam for internal medicine first then dive into applying clinical fellowship in order to find a job in the province-based speciality pathway? Or it's easier to apply for a clinical fellowship first? Thanks for your reading.

The practice eligibility route and the province based specialist pathways are two different routes. You have to do one or the other and not both (thankfully!). You have two options: 1. Practice eligibility route, if you apply to this and get eligibility to write the Royal College Exam in Internal Medicine, getting clinical experience will not be difficult. 2. Enter Canada as a clinical fellow directly in Nephrology & explore Province Based Specialist Pathway. The fellowship is definitely easier to apply for and does not require any exams. However, getting into Province Based Specialist is not easy and depends on factors that I have explained in the video. If I were in your position and knowing so much about the different pathways in Canada, I would explore the practice eligibility route more seriously. I would also simultaneously apply for nephrology clinical fellowships in Canada to get Canadian experience on my CV!
