I'm not saying it's the pivotal moment of the show, but I think it serves to show that Tony deep down has a lot of capacity for compassion -- manifested here in a mother figure that Melfi acts as a therapist (I think she actually says this herself) . Tony is turned into a monster by horrific parenting and upbringing. The fact he still shows shreds of morality/compassion as an adult is actually remarkable (compared to say Janice, who is thoroughly evil).

So, it makes sense she would be annoyed at his visible concern for Dr. Melfi; then soften when she is invited. Carmela’s emotions are very surface level and nearly childlike from all of the years of Tony showering her with gifts.
