He did not kill her mother. Carly often blamed other people (in this case, Tony) for bad things that happened to her often without taking responsibility for her own actions. I think the stress of finding out how Carly took advantage and hurt others killer her mother.

Tony has no heart. Yes - neither of them were responsible for her death but Tony did push because he wanted to make Carly suffer. It makes him a bad guy imo but I don't think he was responsible for her death. But to then make Carly feel worse - I really dislike him and Carly is vicious but at least she's not pretending to be good like Tony.
