That was a close call. Congratulations to The wide awake ATC. PS: I know that we have a GPS linked call-out that tells us that we are entertained runway, and which it is but I wonder whether cameras and a laser system will help ATC in future to know when an aircraft enters a runway. Just a thought.

Perfect example of why you always check you heading against the runway heading before you takeoff. Not to mention that a runway at night doesn’t look even remotely like what was pictured. Great job controller for keeping your eyes on them!

Not to mess up your theory there but the taxiway they "attempted" to depart from was parallel to RWY 22L. The HSI would have been a false confirmation.

@@TesterNA Might be, I didn't look that well nor do I fly out of there. Most runways have a parallel taxiway. Headings will match if on a parallel. Their first big clue was all the blue lights.

@@TesterNA Just looked and you are correct. The taxiway would have been a different heading from runway. You absolutely verify heading as we set the HSI bug before takeoff but the heading will not verify you are on the correct piece of concrete/asphalt. lol

Taxiways have yellow lines in their center, runways don't. This crew could not even look out the window for a second? Also, what's up with the pilot monitoring? Tower had to call him 3 times to cancel the takeoff and he didn't read back to acknowledge until the 4th time. That along with the confusion deserves a write up on the pilot monitoring.

O'Hare is a mess to taxi around, especially if you are assigned a runway on the other side of the field from your departure gate. I'm surprised that the A320 EGPWS didn't warn them that they were on a taxiway. I flew the A300 (a generation older than the 320) and got an "ON TAXIWAY!" aural warning in error at Boston one time. For some reason the EGPWS thought the runway we were on was a taxiway, and when the throttles advanced to takeoff thrust, it started yelling at us. Caused us to abort while we sorted out what was going on. On the second try, everything worked normally. Never did figure out what happened.

The “on taxi way” call is triggered by 40 knots of groundspeed. They aborted at 28 knots, so it wouldn’t have triggered yet.

PLENTY of missed cues, not the least of which is runways do NOT have blue edge lights. They couldn't even use poor visibility as an excuse. I realize that it was late, but that's when you especially have to be on your toes!

Yeah, I don't get it. I don't fly, but watch some of these after Comair 5191 killed a friend / coworker of mine in Lex, KY. When I heard "heading 180" and knew they were supposed to be on 22L, I was immediately pissed that the pilots didn't notice. Not checking the basics like that is BAD. On modern aircraft, why the HELL doesn't the aircraft GPS technology check the heading AND the location vs. where they are SUPPOSED to be, and WARN them? Then if that happens, you don't do squat until it's sorted out. Like with Comair 5191, that tech. wasn't on smaller commuter planes back then.

I believe aircraft are equipped with a sensor that lets the crew know if they are on the correct runway. I’m surprised they didn’t get some kind of warning signal. I remember a crash several years ago in Asia when a plane tried to take off on a runway that was closed for maintenance.

Well, if you're not a commercial pilot and don't know the rules, the lines set-up, the lights set up, etc, that's not surprising. But the commercial PILOTS are supposed to KNOW that stuff and have REVIEWED that stuff BEFORE the flight. How that can happen at a major airport in the US in modern times on a BIG jet with NO warnings from the technology is just completely outrageous to me.

@@rogergeyer9851 that automation has been around. It’s set for 40 knots, because if set lower, it could be triggered during longer higher speed taxi. 40 knots is achieved rather quickly. It seems this particular case, they might’ve started to accelerate slowly. Maybe they had a feeling something wasn’t right. ATC told them to stop and they did at 28 knots, which is barely above taxi speed.

There could have been ground traffic on that taxiway and once airbourne, if they made it that far, they would have been heading West instead of Southwest. They could have had a mid-air with another aircraft in the very busy O'Hare airspace.

It takes many years of airline experience before it is easy to tell. These pilots were probably only making 70K a year. The pilots that are real pros at identifying runway lighting are the guys making 150K+

@@marks6663 I would expect that making the distinction between a runway and a taxiway would be an important part of basic pilot training. I'm assuming that most airports worldwide follow a standardized system of markings and lights, but that may not be so. There are a number of videos of planes getting lost and making wrong turns, especially at night and/or in the fog, so the identification markings must not be consistently clear.

That means the pilot has to call a supervisor at the tower to explain his actions. This can result in an investigation and discipline if the pilot is found to have made an error. Like starting their takeoff on a taxiway rather than a runway.
