I think that managing follow-ups in that way would bring its own set of problems. What if you want to go on the tangent of the follow-up answer you just got? Maybe turning the conversation into a table of contents, have it fixed on the side and be able to type a prompt whatever scroll position you're in.

Think of it as breakpoints that open up the follow-up window for that particular breakpoint. When you add a comment to Google docs, the comment is localized to the “breakpoint” (the point of interest) - effectively branching off to a different follow up. In other words, the follow-up window is not stagnant, but rather dynamic based on breakpoints.
