A “health home” is not a place - it’s a way for providers to work together to make sure people get the care and services they need.
In this Lunch & Learn session, Michelle Varvaro, Community & Provider Relations Specialist for the Children’s Health Home of Upstate NY (CHHUNY), provides an overview on Children’s Care Management Services. These services are part of the NYS Medicaid Redesign and assist families with communication amongst providers, connection to services and resources and follow through with care for chronic conditions.
The presentation also includes an introduction and overview of services provided by the NYS Kinship Navigator from Rae Glaser, Director.
Caregivers often spend hours on the phone trying to find support for kids in their care. With a Health Home, a Care Manager becomes the key point of contact in managing services for the child. Children from birth to age 21 must be enrolled in Medicaid and meet eligibility criteria including HIV/AIDS, Serious Emotional Disturbance, Complex Trauma, Sickle Cell Disease. Other chronic health conditions may also be considered for Health Home support.
