Cytototrophoblasts cause trophoblastic invasion of maternal arteries, therefore converting them from high resistance vessels to low resistance vessels. In molar pregnancy even though the cytotrophoblasts are proliferating , they are the ABNORMAL counterparts , in a way that they forget to cause trophoblastic invasion . Hence those maternal arteries opening in intervillous spaces remain high resistance vessels. Causing PIH

Well, let me try … Its not the cytotrophoblasts that prevent PIH but actually its cytotrophoblastic invasion of maternal spiral arteries that prevents PIH. So in molar pregnancy, since there is placentomegaly, large number of cytotrophoblastic invasions has to occur. So the risk for PIH increases
