Chronic Disease Management through Enhanced Community Care in the HSE

This webinar is for General Practitioners, GP Practice nurses and GP Practice teams, as well as managers and staff working in the Enhanced Community Care Programmes for CH CDLMS (Community Healthcare Cavan - Donegal - Leitrim - Monaghan - Sligo) in the HSE and aligned hospitals. The webinar gives an overview of the Chronic Disease Management Hubs and services which are being rolled out as part of the ECC Programme across CH CDLMS (CHO 1), with a focus on the Errigal CDM Hub in Letterkenny, Donegal.
About HSE
The HSE is the Health Service Executive in Ireland and is responsible for governing and operating public health services for the population in Ireland. CH CDLMS is the Community Healthcare area covering Cavan, Donegal, Leitrim, Monaghan and Sligo, formerly known as Community Healthcare Organisation 1 (CHO 1). CH CDLMS is aligned with Saolta Hospital Group and RCSI Hospital Group who are our partners in progressing the Enhanced Community Care Programme (ECC).
About ECC
ECC is an initiative under the Sláintecare Reform Programme and is about enhancing and increasing community health services and reducing pressure on our acute hospital services. It is bringing care closer to where people live - right care, right time, right place. The programme is being rolled out by the HSE and includes setting up eight Community Healthcare Networks (CHNs), three Chronic Disease Management Hubs (CDM Hubs) and three Integrated Care Teams for Older Persons, in Donegal, Sligo/Leitrim, and Cavan/Monaghan, providing integrated care pathways for our population between acute and community healthcare services.
