Honestly, it just starts with a body type and features you like--face shapes, maybe they're really long and pointy or round or have puffy cheeks or like six elf ears etc--and body features like wings or whatnot, and then you just grow it from there. Nobody really starts from a finished concept unless they've been thinking about it subconsciously for a long time.
