Where/how can one obtain this binding string? These are not readily available in the USA. Where/how can one obtain a mouthpiece cap that fits well over this type of binding?

Some type of Vandoren D model is his mouthpiece. (Since he’s playing a French system clarinet ). The VanDoren models (German sounding ) mouthpieces have the grooves for the strings already made into it.

Thanks for your reply - but he’s definitely playing a German System clarinet, not a French Boehm system.

that's the Vandoren D20 mouthpiece, reworked by S&S (German facing). The reed is Vandoren V21 German cut. You can also use our JSB Gemran cut on that
