To everyone saying this method of extraction is awful, it's less traumatic to the areas around the pore. Using excessive force on the skin causes elastin to break down faster than using this method. Pores will shrink faster than your skin will gain elasticity.

Simple answer is genetics. This person has very oily skin. And the oil clogs the pores and causes them to fill with dirt and debris. This is called a multilocular comodone or as Dr. Lee puts it, a super comodone. Simply put it is actually one giant blackhead with more than one opening. Given how large this one was and how hard it was to remove I'd hazard a guess that it was a multilocular dilated pore or whiner.
