History - 55 year chronic alcoholic patient with abdominal distension 1) Causes of abd distension: Fluid-ascites, Fat-obesity, Flatus, Feces, Full bladder, Fetus-pregnacy, Abdominal mass-tumour 2) Clinical examination: Shifting dullness, Fluid trill, 3) Causes of ascites (SAAG - Serum albumin ascitic gradient = Sr albumin - Ascitic albumin) a)SAAG = > 1.1 ------ Cirrhosis, CHF, Budd chiari syndrome... b) SAAG = < 1.1 ------ Nephrotic syndrome, TB, Carcinoma... 4) Past history/negative history: Jaundice, Hematemesis/Malena - upper GI bleeding, Neurological symptoms -hepatic encephalopathy, Blood transfusion, behaviour change - Wilson's disease, fever , TB, DM.. 5) Signs - Signs of liver cell failure, signs of CHF 6) Causes of generalized edema - CHF, Nephrotic syndrome, hypoproteinemia 7) Causes of Hepatomegaly with Ascites Causes of splenomegaly with Ascites **Causes of hepato-splenomegaly with Ascites - Cirrhosis liver with PHTN (hemochromatosis), Lymphoma, leukemia, Disseminated TB 8) Inv. in aascities : CBC , LFT, USG abd, CXR, Ascitic fluid analysis (20-22G theruptic, 15G diagnosis) 9) Ascitic fluid tapping (best site) - Left Lowe quadrant on spinoumbilical line-medial 2/3rd, lat 1/3rd 10) Colour of Ascitic fluid with cause 11) Complications - SBP, Hepatorenal syndrome, Hepatopulmonary syndrome, Hernia, Pleural effusion
