Phenomenal job as always. I would add an acute demyelinating lesion to the differential diagnosis. Keep up the good work!

Thank you! Agree that hypoglycemia and demyelinating lesions are on the differential diagnosis of acute ischemic stroke. However, they were left out here due to the clinical presentation suggestive of a very large lesion conforming to a vascular territory.

what kind of radiological imaging tools do you "like"? do you use 3d reconstruction, or do you tend to stay more with 2d, or MIP?

It’s not really a matter of “like.” We use anything and everything that will help us make the diagnosis. When it comes to imaging of vessels, we usually use MIPs. They are quick to generate and make detailed images. We use 2D slices to confirm things we see on MIPs. However, when there is a very small abnormality (like an aneurysm), 3D reconstructions become valuable.
