The player is most probably moving way too early. Specifically do the drill in this video where you are only allowed to move after the ball passes the cones

how hard should we hold the bat? should i be crunching the bat with my left had(i'm a right hander) and how tight should be right hand be in comparison?

Usually, the top-hand (left one in case of righties, and vice-versa) is supposed to be held tighter when it comes to most straight-batted shots, similarly, the bottom-hand comes into play when you look to complete a drive (finishing over your left shoulder, again, as a righty), or hit any cross-batted/lofted shots where you again finish with a full swing.
