To explain some of the story, you play as Birthday Man, aka a popular idol known for his Security and Janitorial expertise. The two women in the home at the beginning are actually both his wives, that's why birthdays are so special to him, because he can officially marry the smaller wife when she turns 7. The red-vest lady is his first wife, who is jealous of the new small wife, so she makes silly messes in the building so you will run late and miss the small wife's birthday. This is why in the hospital ending, they look so upset. Not because you had both of your legs amputated, but because you missed the birthday for new small wife.

The machines in this game are a design knock-off of Boss Coffee, a brand that's in machines just like this all over Japan. The logo on the side is similar to what their logo is now, and the picture on the front might be a nod to how they used to have pictures of a stone-faced Tommy Lee Jones on the front.
