Traditionally, ideomotor apraxia has been characterized by deficits in both the imitation and the execution of communicative gestures, notably pantomimes and emblems. Importantly, the expression of deficits in communicative gestures is not limited to response on command. For example you can see problems when the response is prompted by a picture of a tool/object, and the examiner ask the patient to, "Show me the typical use of this tool". Although true that performance is generally much better for naturalistic actions and often indetectable in some patients, severe ideomotor apraxia can also cause disturbances in the handling of real objects and tools.
