Collaborative Care is a model of practice that promotes and prioritizes the highest quality and most efficient patient care and well-being through a coordinated effort by multiple health professionals working together.
At Southlake, we have implemented the Collaborative Care model on multiple units, co-designed by our staff to ensure they have a voice in how they care for their patients. Through this model, our patients and their families play a vital role in their care plan and are at the centre of their care.
