hey. ☺️ yes you need to fill in one sheet. will take you 5 minutes, really simple. & you do it at immigration, across the road from the courthouse. you just need your parents birth certificate that states they are cook island born, and your birth certificate that has that parents name on it. & it cost $25 to do. ☺️ so worthwhile. 🤗🤍
