They're trying to raise awareness of Selective Mutism I believe. Even in emergencies you're unable to use your voice. I grew up with it not even knowing it was a thing until I was 25. It's classed as a disability as it's a debilitating anxiety disorder. In Sam's case brought on by the trauma of his Mum. It's hurtful when people say "you should've said something" after something bad happens that we could have prevented had we have used our voice. Sadly not many people understand and think it's just shyness or being quiet. I think Sam's a great actor.

Does Lily really need an ambulance just for choking for a couple of seconds? I thought you only need medical help if they have to do the heimlich maneuver - and even then you're only supposed to take the person to the doctor's for a check-up to make sure no bones have been broken or anything. At least that's what I remember learning at school when we did medical training! Calling an ambulance when Lily isn't choking anymore AND the country's going through a global pandemic and medical care is super over-stretched seems kind of over-dramatic. (Yes I know it's a fictional show, yes I know I'm taking this too seriously again! Stuff like this always bugs me more than it should!)
