I saw somewhere that the signal tower (which affects everyone's form) gave the doctor some sort of suction cups on his hands and feet. Also, canonically, the tower sucks the personalities, feelings, and traits from the townspeople and then takes their bodies. This is why in the first two episodes of this game there was a lot of clothing that seemed like there were bodies in them before SOMETHING happened. This signal tower also "melts" every person's face, which explains why everyone is SO FREAKING UGLY. I don't know why it doesn't affect Baggie and Sasha though.

Here's my theory, the doctor is the same species of monster as the chef and the customers in the first game. Because when before you first see the doctor there is a room you past through, with a wall of the same masks, in the first game. To further that, the chefs have an animation, where they take of the masks about 25% of the way to scratch there real face. You even egnolaged that animation. PS.
