The joke is that Ivan the Terrible killed or tried killing all his opponents. Which is how he operated in real life. To give you an idea, he killed his first rival when he was 13. He came to power because his family killed each other off until there was only him left. Also, every character's death mimics their real deaths. Alexander the Great was almost certain poisoned (he came down with a fatal illness after drinking wine and his rivals were infamous for their use of poison). Frederick the Great died in his sleep while sitting in his chair. Pompey was assassinated just before he was about to give a speech (his why he never got to rap). Ivan also tries to kill Catherine the Great with a horse and a common urban legend is that she died from having sex with a horse which is why it doesn't work and she comments "That horse story is a pile of shit".
