Why, for 17 years of PAP therapy has no one ever checked how effective my PAP therapy is? Once a year my prescription is renewed by a doctor visit. No questions and nothing advised, has been the routine for 17 years.

Some important points raised. After 18 months use of a resmed machine I've had to figured out a lot for myself. Resmed has been most unhelpful and don't seem to understand rebreathing or deny there's an issue. As for pillows masks, the machine can't tell the difference between a mask leak and air escaping via the mouth so increases pressure to try and deal with a mask leak which makes things worse. Not great for the nasal mucosa.

Appreciate the insights mate. It's true about the 'leak' however ResMed devices do have 'leak compensation' which will actually reduce the pressure if leak goes too high.

I had the same problem with my nasal pillow leaking air but when I switched to a smaller pillow, I rarely have air leakage anymore. Also, your head gear needs to fit properly.

Awesome video, I learned so much! So Resmed Nasal Pillows for people on lower pressure? What is lower pressure, Less than 10? to limit rebreathing? And set your machine to Full Facemask even if you are using a nasal mask! Interesting. I am going to try that. I am going to try the Resmed Airfit P30I. What about the new Fisher and Paykel Solo Nasal Mask? Any info on the venting on that mask?

WOW. My wife has been on CPAP since 2016. I was recently home tested and discovered (?) I have AHI = 28.3. My equipment is expected any day. I ordered the nasal pillow because I knew a full face mask would never work for me - to constricting. So my adventure starts. This talk and discussion was just plain wonderful! I am going to look forward to more closely following your content for sure and this doctor was - well what can you say - I think he was just amazing! Thanks for this information.

Should look into medications and their affects, i was put on statins and my machine couldn't control my apena. Stopped taking them and have now got control back!

Thanks for the video, and showing folks trying [engineering] new things. As a computer programmer, I see many devices where the device is 100% the same, and the software is the change. What a great world (for us) to just be able to buy a one-time flash of the device's program memory. Then everybody with an APAP, VAuto, etc., would be able to get this new algorithm.

I just love this guy. He’s a true scientist questioning the status quo. Very excited about KPAP. I have the n30i which has been kind of awful to use with vcom. It’s noisy even with the 18” hose added. Is that just the nature of the mask?

Just wanted to give you a head up what i found out when i '' WEASELED " our CPAP machines, The New White Foam turns into pieces and dust trying to get it out. Had to Dermal the Air Chamber to get it all out, then i had to clean it out with water and then with the air compressor. Just a heads-up Bud (Mate). I have some pics of it if you want to see them. Glad it's working well for you buddy. The high inhalation airflow does cause a lot of issues for many such as mask leaks, dry nose throat and mouth and aerophagia. It can also some other respiratory issues like treatment emergent central sleep apnea. Cheers

Interesting comments about pillow settings. I use an N20 with 13.6 fixed pressure, but want to go back to the P10 with vcom. He said you could lower the pressure with pillows, however I thought I needed to increase the pressure when adding the vcom.

I actually love the pressure to breath in. Last night I was eating chips and got chip crumbs caught in my airway and I could not free them. I had already emptied my lungs on a cough, Id drunk water.. I could not inhale because my epiglottis was closed.. I was starting to panic. I managed to get a tiny wheeze of air through but not enough. I was already spinning seeing spots, so as a last resort, grabbed my CPAP with a start pressure around10.5mm and put that on and thankfully, it pushed enough air into my lungs so I could actually start coughing to clear my airway. It took a minute or two before the coughing settled down. So if I did not have inspiratory pressure, I probably would have passed out and possibly died. Normally, I cannot even fall asleep without positive pressure. I tried two months ago, and was totally unable to fall asleep.

I'm on CPAP pressure 12 for clear airway apneas (moderate to severe without therapy). Therapy has been very good with full face mask (AHI mostly < 1). Would this new KPAP be worth switching to? I like the idea of having 1/2 the leak.

I have tried the vcom with both the P10 and N20. Both attached between the heated tube and the small mask tube. Found it was much louder on tbe N20.

My pre vcom fixed pressure was 11.2 and 11.4 would give me aerophagia. I have since upped it to 12.4 with vcom and going to attempt 12.6 tonight and find my upper limit.

all cpap machines are made up of the same components (a few minor differences ie. bluetooth, touch screens etc) but the therapy components are very much the same (ie all ResMed devices are the same mechanically but the firmware that controls the blower is different)

The actual device will not be new. All CPAP machines (including APAP's and BiPAPs) are essentially made of the same components. The only thing that really varies is the firmware that analyses the respiratory data and controls the speed of the blower fan. This is essentially new firmware that will be installed on devices already on the market. Cheers

That's a big generalization and clearly incorrect. If you've tried a CPAP it's either not titrated correctly, or not the correct device for your particular sleep-disordered breathing.
