CPT® E/M Office Visit changes: Using medical decision making to document an office visit

On Jan. 23, 2020, the AMA hosted a panel presentation to help the health care community hit the ground running when the evaluation and management (E/M) office visit documentation and coding changes deploy on Jan. 1, 2021. This was an awesome presentation. I have a question when you say there needs to be documentation in the note of the problem do you consider continue meds appropriate documentation?? What needs to be documented in the note

I was a little confused at 32:30 : so prescribing ACEI is low, but prescription drug management is moderate, shouldn't both of them be moderate?
