Current diagnostic criteria exclude many pediatric patients with probable migraine

Christina Szperka, MD, FAHS, Children’s Hospital of Philadelphia, Philadelphia, PA discusses developments in pediatric headache diagnosis and treatment monitoring. She highlights data from a recent cluster analysis looking at the underlying relationships between 11 migraine-associated symptoms, which aimed to critically evaluate current diagnostic criteria. It was found that nearly 60% of youth with probable migraine did not meet the diagnostic criteria, despite experiencing multiple migraine-associated symptoms. Notably, allowing for photophobia or phonophobia, instead of requiring both, recategorized 55% of youth as migraine who initially did not meet the criteria. Common patterns of symptoms should be considered for future revisions of the diagnostic criteria. This interview took place at the American Headache Society (AHS) Annual Meeting 2024 in San Diego, CA.
