When you're tamping the grind, don't twist the tamper too much. A simple single twist and lift is enough. I think that's why some grounds were loose when you lifted the tamper.

Maybe add a bit more air to the milk? I see the pressure isn`t optimal, but u need to introduce some air to get that microfoam.

I think you forgot to spray your coffee beans before grinding to avoid static buildup
