Actually, it's not quite either. It's a core part of Thanos' character. He doesn't believe he has the right to choose who lives and who dies. He could've used the snap to destroy his enemies and save his allies (like Iron Man does at the end of Endgame), but he makes it totally random for the same reason he spares the lives of the people he fights whenever he can. It's the twisted morality that lets him commit genocide and still believe he's a good person who's saving the universe and in my opinion it's what makes him such a compelling villain.

To this day I still do not understand why the directors thought to change Thanos' look from guardians 1 and Avengers 2. It's one thing to change his whole purpose for living, it's an unforgivable one to make him just look like a buff purple Josh Brolin as opposed to in the other films. First off, do you remember that Thanos has FIVE out of six infinity stones. He can easily block or absorb the attack and it wouldn’t matter much. Secondly, Vision is significantly damaged meaning he can’t use his powers as well. This isn’t Age of Ultron, Ultron was heavily damaged meaning Vision could easily beat him.
