if you look close, you can see cloth lines in the back so green screen i believe the eye chart is part of the set, however

Early Friendly Doctor videos used a green-screen and they weren't great. Especially with edges going in and out of focus glowing green. What Fred's wisely done here is print an image of the office on a large bit of PVC which he can fold away and hang up whenever he does the doctor. You can see the creases in the background and when he shines the light on it, it reacts like a flat image (because that's what it is).
