First time might be, solo, for the story, then in party for 'farm'. And if you actually paid attention, we already have another Prime Evil in a Dungeon (Baal). Lorewise, in the franchise, Mephisto, the other, mainly comes in D IV, with her daughter Lilith upfront (maybe in the lore he himself will show up).
