No it means if you got hit by any bosses attacks, not crashing into walls, or the collision.

You really can't get hit, you get stunned like Yoshi in Yoshi's Island, the ball is tied to your lifeforce, if it falls off a pit or breaks from an enemy you straight up die, you can roll the Crystal around to score more points for a Risky challenge run and you can also kill yourself for the humour if you slam the Crystal on the ground.
