The tilt is not towards the sound eye, it is towards the affected side, right?? Here the red images are tilted and it is towards right side that corresponds to the right eye.

@@LaSalviaBodysurf as we are dealing with third nerve palsy, the inferior oblique is also affected ( responsible for extorsion!) . However the superior oblique is intact supplied by 4rth nerve ( responsible for intortion) . Therefore we see an intortion tilt . :)
