Dizziness in the Emergency Department - 32nd Annual EM & Acute Care Course

Using primarily the Emergency Medical Abstracts (EMA) database of over 17,000 abstracts, 28 presentations, each of 30-minute duration, are presented along with four 90-minute faculty panels. The focus of the course is the new, the controversial and the provocative. The course faculty synthesize the literature and combine it with their clinical experience to provide participants with specific recommendations regarding diagnosis and therapy related to emergency care.

Congrats for the great presentation ! Although I have a question. I understood that in lateral gaze, (26m08s aprox.) no need of CT scan. But if in the lateral gaze, we have bidirectional nystagmus ? Is this not central cause to exclude ? Thanks a lot for sharing your experience !

Lateral alternating nystagmus is not peripheral in origin. Always central and we then do test of skew as well

Stigmatizing, shaming bias. Open-minded, scientifically trained experts would be interested in possible co-morbidities contributing to both depression and acute onset of dizziness. They would want to follow best diagnostic testing, given ER understaffing and overwhelm. At least make referrals to external (non ER) ENTs, neurologists, and cardiologists, once obvious hemorrhagic and ischemic stroke and major cardiac pathology is ruled out. At least look into the external ear canal, the ocular fundus, and test the cranial nerves
