It's simple, the Home Body/Shut in types must have a vitamin D deficiency from lack of sun or other means making their bones weak. The obvious solution is for them to do a bone density test to make sure it's safe for them to go outside on the regular. Or the vicious cycle of going outside, injuring themselves, then being forced to stay inside to recover will continue.

In the us we could be bleeding out and would rather walk to the hospital than pay for an ambulance actually we would try to patch our selves up over going to the hospital, it's super expensive

Well someone has to pay for the various resources and services involved so you either pay directly (either with your own funds or via insurance) or get yourself the "free" midel where your taxes go up and the government supposedly uses the money to staff and equip the emergency services (how much actually gets to where it's needed depends on the amount of corruption in the ministry/department that handles health related matters). Neither is great but at least you'll actually get seen in the US, while countries that go for socialised health care have trouble staffing their hospitals and might give an appointment for a check up in a year or two.

It could have been broken or fractured unnoticed, in which case as little movement as possible is heavily advised. An ambulance is a perfectly reasonable thing to call if you don't have someone to drive you.
