Physical therapy evaluation of the lumbar spine using a McKenzie method has a few distinct parts. First we discussed the patient's reason for seeking physical therapy evaluation in their specific goals. During that time discuss when it started having problems at their lower back and hips and legs. We like to know if there is a suspect problem that occurred at the time or move interactivity that corresponded with their problems. We also review a prior testing such as an MRI, x-ray and/or blood work. If you have received treatment from another healthcare practitioner we want to know the results of the treatment and intervention. We often learn a great deal about the patient and how he may respond to treatment by also assessing concerns and fears. After a lengthy discussion we then form a lumbar McKenzie method evaluation which attempts to identify if there is a movement or position that reduces or eliminates the patient's pain and/or functional limitation. If we're concerned about true neurological deficits will also do an assessment of the strength loss, loss of sensation in the lower extremities or changes in the deep tendon reflexes. Once we begin the examination we see how different movements in standing in in supine may change and hopefully improve the patient's pain levels. During the can the evaluation where carefully monitor monitoring how the pain is acting if it is decreasing in intensity or changing location. The patient's feedback is integral to understanding how to help them. If we see a clear increase in range of motion and possibly a reduction in the pain symptoms during the first visit it is a very positive sign that they will benefit from this approach to treatment. In cases at the patient has a clear sign of a directional preference which means they had a gross increase in range of motion or significant reduction in pain we instructed patient to perform the desired movement 10 times every 2 to 3 hours over the course of 24 hours. In some cases I wait to three days before I reevaluate depending on a case-by-case basis. We have sent seen success with patients with lumbar disc herniations, lumbar stenosis, lumbar degenerative disc disease, lumbar degenerative joint disease, lumbar arthritis also known as spondylosis, lumbar strain and sprain and especially sciatica. Our averages number visits is 6 to 8 visits which we provide our patients with a home exercise program. Within 2 to 3 treatments we typically know if we can reach the patient's goals. At our clinic in Paramus New Jersey at apex orthopedic rehabilitation we see tons of patients with spine related problems at the cervical, thoracic and lumbar spine region. Oftentimes these patients have not responded to other courses of treatment. Are perfect patient is highly active in treatment and compliant with the simple home exercise program. In addition wins instructor patients to not sit more than 30 minutes at a time and to use lumbar rolls to maintain a more upright posture when sitting or driving. The nice thing about the McKenzie methods approach to treatment is that you're only performing one movement pattern and it becomes quickly evident if this'll be a moving that will help you. The most typical movements that help patients are standing lumbar extension and prone lumbar extension. Most of the times patients stop the movements when they have significant pain at the end of the movement even though they don't have more pain one stopping. This is one of the most common reasons patients don't respond to McKenzie method exercises. If you continue to also stretch in other planes of motion of the lumbar spine this will nullify the effects of the exercise the physical therapist has given you.

I've been suffering from major discomfort wrapping around from my thoracic spine for about half a year now, went to some physical therapy which helped maybe 30%, but then I ran into this video and applied some of the exercises and it's really made a difference for me Still trying to get myself 100% "normal" but just wanted to let you know I appreciate all of the detailed explanations of why people have these issues and why these exercises can work, that's something that I sorely lacked in my experience with Therapy, and wish that I came past your channel sooner!

I find that doing this movement causes tingling in my feet..I’ve been suffering with this problem for 2 years now. In the beginning it was the classic shooting pain but now what left in continual burning / tingling of the ankle and left side / top of the left foot. Also persistent pain in the sacroiliac joint on the left. Certain exercises make the tingling intensify and unfortunately this is one of the movements..another is doing dips from monkey bars.

Sorry to hear. Stop anything that causes pain to increase.. You may Benefit from a full physical therapyevaluation.

I did McKenzie lumbar extension for few days and my pain gone away but then i stopped exercise pain came again so i did McKenzie exercise again but this time lumbar extension not working 😢

Try the side glide on the wall. Notice that your pain may be more on one side. You must bend to that same side with the side glide

You need a evaluation to determine cause. Can be a fracture in lower back, herniations or nerve compression. Too hard to evaluate Without in person evaluation

Okay, I cannot find any PT site that addresses low back pain with intermittent sciatica that specifically comes from SEVERE muscular tension of glutes and other muscles in the low back and buttocks. There’s NO range of motion inhibition, in fact there’s hyper-flexibility. Why doesn’t anyone address severe muscle cramping that causes low back pain and sciatica that attacks both sides/legs intermittently or together? Have you ever considered the fact that some FOOD sensitivities can cause a flare up, too? Regarding McKenzie Method?

Sheila Smith absolutely food can do that. I’ve had a few patients that eliminated dairy and reduce or abolished their back pain

The McKenzie is checking for a mechanical choirs of lower back pain. If that is ruled out then you have to look at the blood test and other causes of your back pain

Sheila Smith a mechanical diagnosis means a position or movement, exercise will improve your condition. In some cases people suffer from lower back pain with a autoimmune problem, organ, tumor, severe inflammation and will not benefit from this approach

Hi Sheila, I experience similar problem that you described. For 3 months, I have low back pain, kind of stiff lowe back with radiating to my left leg. I guess it might come from the msucle tension but I don't know how to diminuish that tension.

Yes. At a certain point once you have adequate range of motion you can do it as needed or once a day.

Thanks for your reply… Would the increased tingling sensation definitely indicate further compression of the nerve then? I only ask because when you visit the dentist and get an injection in the mouth, as the anaesthetic wears off you get tingles which intensify. I wondered if it could be the same and if actually intensifying in tingles meant the pressure was being relieved..

Typically it indicates further compression unless you previously had numbness sometimes it is an indication of a improvement. Wish You were New Jersey so I could give you an evaluation.

Why bash chiropractic physicians in the video? Maybe the same reason you don't think ordering an MRI is necessary in most cases. How is it possible non invasive clinical information on the area your treating with physical medicine can be a bad idea or not necessary? Oh I know why...because physical therapists scope of practice does not permit them to prescribe any diagnostic tests never mind an advanced test like an MRI...I have some news for you and it's easy to understand. Non invasive tests like MRIs are never a bad idea. Never.

@@whatthe6532 MRIs do not use radiation. They use magnets. This will sound combative. Not my intention. Even physical therapists that get there doctorate degree do not have the ability to write diagnostic tests like MRI or even X-rays. Non invasive clinical tests are never a bad idea. Luv the technique McKenzie developed and use his exercises daily with my patients. So please don't read this as anti McKenzie. It's anti not thinking an MRI is necessary. Again , non invasive clinical information is never a bad idea.

@@JohnPandolfo121 I dont think he was bashing MRI's, I think he was stating it wouldn't change his plan of care because he's treating based off symptoms. MRI's can catastrophize patients pain and sometimes show results that aren't the true cause of pain. Im sure youve heard the stat that between 20% and 76% of people who don't have symptoms will show signs of disc herniation on an MRI.

@@meredithzoltak1578 Back to my original point. Clinical information that's non invasive can only be helpful. Every physician that reads scans adds in the findings of the examination. Of course there can be findings on MR that can make a bad physician treat the wrong area. Good physicians add into his or her interpretation of there final working diagnosis. Best example i can give you is lab work that shows normal blood Magnesium levels. Your blood will steal magnesium from every other place it belongs to keep your heart beating. Non invasive scans are 100 % of the time helpful. Not 99.9999%. As for him not bashing MRIs. I disagree. He was clear how they can cause physicians to misdiagnose there patients cause of pain. Wrong !! Only bad physicians will say an MRI can cause a doctor and patient problems. It's borderline absurd.
