I know for direct hyperbilirubinemia no role for phototherapy but some expert neonatologist started phototherapy in an extreme preterm with direct cholestasis with impaired liver functions is that right??

Hello! This is a great question! Remember that even if the direct bili is high we are still worried about a high indirect bili crossing the blood brain barrier. So for example if you have a 800g baby who is 5 days old. If the total bili is 20- even if 8 of it is direct- that means the indirect component is 12 which is too high for this premie (especially as direct bili can also displace indirect bili from albumin and make it easier to cross). Does this make sense?

Just that to you can use the timer on the bed to time an event or a process (so if the infant is having a seizure for example). This was a tip from what of our viewers :)
