It's actually a really neat application of physics: the part of the stethoscope that is placed on the skin is called the diaphragm. Sound waves that are produced inside the body (like the sound of heart valves) make the diaphragm vibrate. The vibration of the diaphragm transmits the sound into the hollow rubber tube, which then leads to the ear pieces. The amplification happens for 3 main reasons: 1) Without a stethoscope, sound waves from inside the body would travel outwards in all different directions. The diaphragm 'collects' all of these sound waves, and channels them in a specific direction (i.e. up the rubber tube and then to the ear pieces). 2) The size of the diaphragm is much larger than the opening of the ear piece. This means that the vibrations of the diaphragm are funnelled to a small area within the ear piece. 3) The ear piece is placed very close to the ear drum so that the sound is not lost to the environment. EMT Here. Use the stethoscope all the time since in a field setting we don't have a lot of other tools. Mostly for taking blood pressures and auscultating the lungs (and occasionally the upper airway), but it is pretty much useful for anything in the human body you can hear. I've seen a salty old paramedic use it to increase their index of suspicion for a tib fracture by auscultating one end and tapping the other with the handle of their shears, for example. They did put it in the report and it caused... some discussion to say the least! A review of literature showed that such a technique was dated but valid

I think you're probably referring to the bell of the stethoscope. The bell is better for low pitched sounds that may not be strong enough to vibrate the diaphragm. This can be useful for certain heart murmurs that are difficult to hear (eg diastolic murmurs)
