My wife recently (about 12 weeks ago) completed a micro discectomy with Doc Donnally. It worked! She had butt pain that was pretty intense. This pain did move down the leg as well. It was bad. We really like Dr Donnally and are grateful for his skill and surgical experience. After the surgery she was active (walking around) a couple of days later. The pain from the surgery diminished greatly in about a week. Minor pain lingered for about 6 weeks and was gone for the most part after 8 weeks. ❤😊

It’s hard to make sense of it all as a patient. I don’t understand how someone can have God awful 24/7 intractable pain but no surgery is offered.

@@DonnallySpineConsult Unfortunately I don’t think you can for neuropathy maybe I’m wrong and I would love to be. Ortho, neuro docs tell me nothing they can do I have DDD stenosis and herniated disc that receded.

​@@noexp7710I went ortho first, got discs removed and cages, L4/L5 & L5/S1. My discs were torn but 6 months after surgery and the insurance companies denying more surgeries, I'm probably going to have to lawyer up. Nearly 15 years later I'm waking up with a gutteral scream everyday and I'm already on so many meds. I'm so over it I can tell you that much

I told my PCP on February 2 that my back pain was unbearable and I was ready to do something about it. He sent me for an X-ray and offered options of physical therapy, see an orthopedist, or massage therapy. I did P.T for several months. Ortho injected cortisone and sent me to pain management doc who did more imaging and a tree block injection. None of these helped so he sent me to a brain and spine surgeon. August 2, Six months to the day later I had surgery that relieved the pain. I got my life back. I can’t but help to think that all the hoops I had to go through were necessary for Medicare purposes. 2:30

Exactly what I'm going through,I have medicare.ive had to jump through hoops the last 18 mo​. I'm finally seeing a neurosurgeon the end of September.in the meantime...ive gotten worse. 😢@@janetaro

You are amazing! Thank you so much for taking the time (out of your very busy schedule) to educate us. It makes such a difference. Doctors don't usually have the time to explain spinal anatomy and possible spinal interventions. I've learned a lot from you and now know what questions to ask.

I was having an emotional meltdown bc med transpo didnt show up for my epidural/steroid injection. Dr couldnt find area to give me 1 last time so I was so hoping for Xmas I could have pain relief. Then u just popped up. I know of 5 patients who killed themselves bc pain meds were cut back & they didnt have u on U Tube. Used to volunteer with Vets & saw 2 Great Danes with spinel stenosis who had spacers & they were 100% better. So guess every1is diff. I busted my shoulder bc I can no longer walk with spinel stenosis bc of pain. I have Kaiser & so far been told I dont need to use my arm or have sex, etc. Im a tennis junkie. Willing to give up marathons but....Thank u so much. I had just given up all hope then ur video came on. U didnt mention spacers or sciatica cushion that alleviates disc pinching nerve when sitting. Leg wont go numb or electrocution while driving. Texans used to be the most hospitable ppl but guess transplants have chged all that. Ur gun violence scares me. I hate pain & when I had fenanyl patches nobody wanted to rob me of those. I was on those with methadone & got an epidural so stopped taking them bc I had no pain. Whoops! Cardio saved that boo boo. Im afraid of fusions but knowledge is power & u just saved my furbabies from wiping my tears. Thank u so much. PTs all say I need shoulder surgery. Sad I think of Tx as Dr Dunsch (sp?) Drug addict spine Dr so thx for giving Tx better name. Stay Safe & Healthy💜🙏💜

Your videos, demonstrations, explanations, and presentations are invaluable. Just excellent! But best of all was the hilarious explanation of the effect on hearing when you are married and/or have children. Wish you were closer to the Outer Banks of NC!

I had a faset block done in May this year and it was successful the nerve pain is gone. So many people said that it won't last long but it is almost mid December and I still don't have that debilitating pain. I had some friends praying for me could this also be part of this relief I am experiencing. I just want to say thank you for your very informative podcasts 😊

I have Chronic Thorastic back pain under my left shoulder toward the spine. Using my left arm gives me SCREAMING PAIN for at least 3 hours. Suffering w/ this daily for over 6 years.

Good luck getting pain meds. I’m 76 with severe spinal stenosis. My last epidural - the doctor shot it in the wrong place. I was paralyzed for 2 hours. He then said SURGERY - fusions, laminectomy, etc. I saw a different surgeon who said an ablation is the way. In the meantime, I’m in agony. Cannot take NSAIDS or Tylenol due to kidney & liver function. NO to any meds. So, I’ll be going to Mexico again for pain meds. How awful we seniors are told to suffer. I’ve had 18 surgeries since age 28. I have a bad total knee replacement & I tripped going to apt, fractured same leg. I’m sick of pain, so if I die from fentanyl in the meds from Mexico, so be it. I just hope my family will tell everyone why.

I’m 74 and recently diagnosed with foraminal lumbar stenosis with spondy present. Is it your opinion to correct with surgery sooner rather than later, given my age?

Thank you ❤in advance for all your info and videos. Currently I have had a L5 S1 fusion. Currently experiencing 🦶 toe numbness (loss of sensation) it’s been a year and a half , do I have hope? In addition when doing a spinal fusion what determines what surgery to proceed with? Tlif, alif? Plif? Thank you so very much. 😅😊

Hi Dr.! I would like to know about this situation of my dad, he has no lower back pain at all only on the lower calf. He can touch toes standing up, the pain is only on the right calf ? Some say it’s piriformis ? Would you suggest an injection for the pain but we would like to avoid an epidural. What are your thoughts vertiflex procedure ?

I was diagnosed with L3/4, L4/5, L5/S1 disc bulge and stenoses. I also have retrolisthesis and mild scoliosis. I had a steroid injection 3 months ago which thankfully got rid of the piriformis pain. I did not have pain radiating down my leg. Now, I am experience groin pain which I didn’t have before. I have an appointment for a second opinion, but was curious as to your thoughts on the groin pain.

Dr. Donnally I really need some advice. I have horrible horrible low back pain mainly on the right side. My situation is a little tricky though because I had an extensive dvt back in 2022. I ended up having 2 surgeries and 2 stents were inserted. I’m confused because I started having this back pain a few months before I knew I had a dvt. The vascular surgeon put the stents along the iliac vein I think. I was had an mri done and they say the pain is caused from the last disc down in the low back. I’m going to pay $20k for disc laser surgery to fix the disc herniation. I don’t want to pay all that money if this is caused from the vascular issue. How do I know? The first orthopedic doctor I saw didn’t think it was related to the vascular issue and my vascular doctor said it isn’t caused from that either. I did see on the mri with my own eyes the disc. If you could offer any advice I would really appreciate it!

My lumbar thecal sac is half way pinched on L4/5 from a hurniated disc 5s1 is severe arthritis. I had a bilateral steroid injection. It went from 7 to 9-10 pain.. Loss bladder bowel control. Burning aching groin pain. Pain from right side back around entire hip. Cant even touch it. All lumbar disc are bulged. Now the worst problem is moderate to severe stenosis c4-7 c4-6 are flattening my left spinal cord from two hurniated discs both legs numb tingling. Left side extremities worse. Im left handed. I got two surgeons who gave me a different surgery plan. A two level fusion and a two level disc replacement. I dont know what decision to make. (This was from falling on ice at work) I dont know what to choose. Thank you for for explaing things. Im still confused and scared. 😢

Not sure, sounds like you’re in pretty terrible shape and have made time to see two experts. Both want to help you. Seems like it’s pretty bad and you have a great options of relief.

It’s me again; I have a SCS, not much help. Have had everything possible done except Surgery. Just don’t know what to do for this Horrific Thorastic pain. It goes from under my left shoulder near the spine and runs to the front of my body under my ribs.

I have had lumbar spinal stenosis for years, probably from L4 -L5 scoliosis. No nerve entrapment. Just localized lower back pain when standing for over 15 min. Can walk the grocery store with a cart with no problem and can do a treadmill for 30 min as long as I hold onto something. Walking without any aids, I can go about 20 min before I have localized pain over the lumbar area. I am 74, and my orthopedist feels as long there is no nerve entrapment, no need for surgery. I am fine with that. Do you ever have an age limit where you would not do surgery? Just curious.

Hi Dr Chester...I have spinal stenosis of L4 , 5 and S1 which has greatly affected my ability to walk, can't walk a longer distance without pain in my back. I feel like my doctor is dragging his feet. I am taking meloxicam, and gabepentin...without these medications I would be in agony. We tried one injection which did not work. Going to try again to see if they can get a result. Is there any surgical intervention that would fix this problem and give me back my life?

I use ICES on my L5-S1 area (spondyliosethesis) and it works wonders. Used it on my hips many years ago. It was. Created by Robert G. Dennis who also has a KZbin channel explaining the technology if anyone's curious. It decreases inflammation substantially. My nerves are still pinched but the pain is much lower. Goes to show that inflammation really is the culprit in many cases.

Currently, I have sciatica pain cause by a herniated disk L5-s1 and also spinal stenosis, some chiropractors are giving a lot of promising results using a decompression table that they have, do you really think a decompression table like the model DRX9000will help L5-s1 herniated disk sciatica nerve pain with spinal stenosis..

I have tingling in my fingers and burning in beforehand but not in my thumbs, and some shooting pains in my biceps for the last two months, got a MRI and X-RAY and a CT scan say my C-5 C-6 C7 , Can physical therapy or Acupuncture help?

@@DonnallySpineConsult do you recommend lyrica to your patients? I know it takes away the nerve pain but my kegs still feel like they are very tight elastic bands. I worry that I may be doing further damage to my stenosis when I don’t feel the nerves.

What are your thought on this MRI result? Is surgery the best option? If so, what type? Variable degrees of disc desiccation and loss of height from spondylosis. Scattered facet arthropathy. Details by level: L1-2: Unremarkable L2-3: Facet hypertrophy. No stenosis L3-4: Disc bulge and facet hypertrophy. Mild spinal canal stenosis. Mild right foraminal stenosis. L4-5: Disc bulge. Superimposed disc extrusion measures 0.9 × 0.9 cm transversely and 2.2 cm craniocaudally, extending inferiorly from the parent disc essentially in the midline. Moderate to severe spinal canal stenosis. Most of the CSF around the cauda equina has been displaced. L5-S1: Posterior annular fissure. Tiny left paracentral disc protrusion. No significant associated stenosis. Conus terminates normally. Included soft tissues are unremarkable.

Hello, your explanations are extremely detailed, very informative and helped me understand the severity of my spinal stenosis. I am 72 years old do you think surgery is a good idea? Last year’s MRI report : L3-L4 diffuse asymmetrical disc bulge with severe thickening ligamentum flavum and fact joint arthropathy, more on right side, moderate canal stenosis. Partial effacement of lateral recess on both sides. No narrowing of neural foramina. L4-L5 mild anterolithesis of L 4 over L5 measuring 0.4 cm from L5 posterior superior endplate. No clear spondylolysis . Reduced intervertebral disc height and T2 signal. Left asymmetrical diffuse disc bulge effacing the lateral recess more on left side and touching decending nerve root of L5. Moderate narrowing of left neural foramen plus thickening of ligament flavum and Severe facet joint arthropathy. There is significant spinal canal stenosis. Rest of the spinal canal is adequate on all levels unremarkable conus medullaris and visualized portion of spinal cord. The conus medullaris terminates at T1. Please can you advise on what I should do? I have severe pain in right leg due to sciatica. Takes me half an hour to get out of bed, it’s so severe. Walked a bit much like 15000 step on recent holiday and sat almost 10 hrs for travelling.
