The smoke is Mito one of his personality who is trying to come to the front because Insanity another personality and the one who also is in the mv is someone who goes to the extreme and sometimes put them in danger so in real life Mito mostly comes at the front when Insanity goes too crazy he's the protector of Ian consciousness in the medical field we call it a : "Persecutor Alter"

It's also good to point out that Mito and Mr. Insanity also represent Christian's High and Lows in Manic episodes, he mentioned Mr.Insanity is his Mania Highs and Mito is his depression state.

@brendatrevino6659 yeah because it's a trigger in the DID system when he goes manic low it will trigger Mito and when he goes manic high Insanity but Mito and Insanity only exist because of his DID not his bipolar disorder but yeah he says that so he can know when he is in a manic low or high but they also can be trigger to front by other things
